Crave Conference is a conference for Middle School and High School Students hosted annually on the last weekend in October at Faith Christian Church in Mauston, WI. Each year we host students from a number of churches around the state, as well as our own church/community!

Permission / Liability Statement

In the event that the attendee needs medical attention, I authorize the leaders, staff, and/or volunteers of Faith Christian Church to seek and consent to any necessary immediate medical attention they may need. I understand the attendee will be traveling by bus to Devil's Lake State Park in Baraboo, WI. I give permission for the attendee to ride in the bus, as well as any other vehicle that the leaders of FCC deem necessary while traveling to and from this portion of the weekend. Additionally, I understand the attendee will (have the opportunity to) hike around Devil's Lake State Park. Finally, I release Faith Christian Church of Mauston Inc. from any and all liability during this conference.

Liability Statement

This conference includes a field trip to Devils Lake State Park in Baraboo, WI. We require all group leaders to collect and maintain on hand permission slips from all students attending Crave Conference with your organization, releasing Faith Christian Church of Mauston Inc. of any and all liability. We also require that you collect and maintain on hand all necessary medical releases and emergency contact information. I understand it is my responsibility to inform my student's parents/guardians of all aspects of this year's Crave Conference, including said trip.
